Chiari Fund will use your personal information to respond to your requests for information and/or assistance. Without your express written consent, your personal information will be shared or shared only to fulfill your request. Your request for assistance is made through an application. All or part of your information is provided to the board members of Chiari Fund for determination of eligibility and appropriate assistance. When reviewing your application, you, you may be given the option of submitting further information that may be beneficial to your case.
In the event that we feel a third party or other organization may be able to help with your request, you will be contacted and required to sign a Release of Information form that will be retained on file with your original application.
When reeving application, Chiari Fund may elect to use technical information, which does not identify a specific individual. This information sometimes referred to as “non personally identifiable information”, may include your personal medical or financial information, or the assistance we were able to provide. This technical medical or financial situation or the assistance we were able to provide. This technical data is used for grant applications and to update previous and potential sponsors as to the use of donations received.
Is My Personal Information Shared?
Unless we have your express consent, Chiari Fund will not share your personal information with any third party unless the following provisions are met. We may be required to provide personal information in response to a valid court order, subpoena, government investigation, or to legal inquires. Unless prevented by law, we will notify you prior to the release of such information. We reserve the right to report to law enforcement personal information, should we feel this release is necessary to protect the rights of others or Chiari Fund and its board members.
Accessing or Updating My Information
Chiari Fund wants your information to be accurate and up to date. If you would like to review what is in your file or update your information, you may contact us with your request in writing or be email and a copy your file will be forwarded to you.
How Long Will My Information Be Active?
We believe it is important for privacy and security to delete or destroy information when it is no longer needed. Unless you are under consideration for assistance, have updated your information, or reasons we may feel necessary to retain your application, your file will be deleted and destroyed 3 months upon receipt. Therefore, it is extremely important you keep us updated on your situation if assistance needed is ongoing. You may request to have any personal information you have submitted to Chiari Fund be deleted or destroyed. You may contact us by mail, email, or phone of such a request. Steps will be taken immediately to have said information deleted from your file.
